Dr. Jason Ryan, co-director of the Heart Failure Center at the Pat and Jim Calhoun Cardiology Center and a member of Qualidigm’s Communities of Care Education Committee visits News 8 recently to talk about the Communities of Care initiative which reduces preventable hospital readmissions for patients with heart failure.
