Putnam Primary Care Providers Join UConn Health

PUTNAM, CONN. – Accomplished primary care providers Dr. Dominick Roto and physician assistant Matthew Sleboda are now part of UConn Health.

Patients will receive the same high-quality, personalized care at the same location at 145 Pomfret Street in Putnam, backed by the resources of the region’s leading university hospital.

As adult primary care providers, Roto and Sleboda offer preventive care such as vaccines and physicals; help patients manage chronic conditions; and offer advice on nutrition and work/life balance.
